
1. A person interested in obtaining a certified copy of a death certificate must first fill out a death certificate request form*.
2. Sign an
authorization
form* in front
of a notary public. Relatives of the deceased person can request the certificate.

3. The person requesting the certified copy must send us a copy of two (2) photo of their own identification cards (such as driver’s license, passport, employment i.d., etc.)
4. A postal or bank money order in the amount of $15.00 for each certificate requested. (No personal checks accepted.) The money order must be made out to the "New York City Health Department."
5. A second postal or bank money order in the amount of $75.00 for the first certificate requested and $40.00 for each additional certificate requested. The money order must be made out to " José Luis Torres, Esq."
Once we receive the five (5) items:
a. a completed death certificate information request form*;
b. authorization form* properly signed and notarized;
c. copies of two (2) picture identification cards;
d. a postal or bank money order of $15.00 for each certificate requested made payable to the "New York City Health Department"; and
e. a postal or bank money order for our fee as shown in paragraph "5" above,
we will immediately apply for the certificates in person. Death certificates are sent directly by the Department of Health to the person requesting the certificate by regular first class mail. If you want them sent by "Express Mail" you must include an additional fee of $10.50 payable to the "New York City Department of Health."
Note: If the person requesting the death certificate is not a relative of the deceased person, a court order is required in the form of a subpoena requesting the documents (such as for a divorce or inheritance case). In such a situation, you send us the court subpoena with the fees noted above and the authorization form and we will submit them to the Department of Health for processing. The death certificate will be sent directly to the court listed on the subpoena by the agency.
